HR Starter Kit for Employers - All Employers

This Kit is a “HR compliance must” for any small business. It includes the following forms and documents:

  • Application for Employment (Since the Application is sold separately as well as part of the Starter Kit, see the separate listing of this product for a full description.)

  • Attendance Calendar – This is a simple 1-page Excel sheet for manually or electronically recording hours worked and/or paid/unpaid time off (e.g. Vacation, Personal, Sick, Workers Compensation Days, Training Days, etc.). Most of my clients use this form to record days off, employee lateness, Workers Comp days off, and other non-hours worked issues. The sheet is set up to allow you to view an entire year of activity on one page. For future years, all you need to do is change the dates from year to year without having to buy a new form each calendar year. You can even setup this form on a fiscal year format, rather than a calendar year.

  • Employee Counseling Record – Whenever a supervisor counsels an employee to improve behavior, comply with policies, improve productivity, etc., this form will memorialize the meeting. In the event of a subsequent termination of an employee for any reason, it is important to have documentation in an employee’s file of counseling sessions.

  • Employee Emergency Notification – This is a non-confidential (i.e., no personal identity information such as SS# or Date of Birth) form that can be distributed to supervisors that includes contact numbers, cell phones, email addresses, etc. for contacting an employee or an employee’s family or significant others in an emergency situation.

  • Employee Personal Data – This is a confidential form containing important personal identity information about an employee that is valuable information when preparing census information for insurance quotes, verifying insurance information, filling out Workers Comp forms, etc.

  • Employee Status Change – This form is an absolute must as a formal document for a supervisor to record any change in an employee’s status such as a promotion, a merit increase, etc. This completed form is placed in an employee’s permanent record and a copy of the form is sent to Payroll. As these forms accumulate in an employee’s file, they provide an invaluable historical record that can be reviewed when issues arise.

  • Employment Links – This document provides money savings links to websites to download FREE Employment Forms (I-9, W-4, W-9) and FREE required State & Federal Employment Posters.

  • Exit Interview – Everyone is interested in opinion surveys today. And what better way to gather important information about your organization, than an exit interview with an employee who has resigned. Usually, an employee on his/her way “out the door” tends to be more forthcoming than people who are still employed. As such, this form takes advantage of this inclination to “speak openly” of an exiting employee by providing 15 important questions that can help you improve the management skills of a supervisor and performance of your organization.

  • Expense Reimbursement Form – This Excel spreadsheet automatically totals the mileage and expense data for reimbursement to an employee based on the guidelines you set. It has a space to input the IRS mileage reimbursement rate that you want to pay as well as a section for an employee to describe any expense item that is recorded.

  • Leave of Absence Request (non-FMLA) – For an employee who does not qualify for a leave of absence (LOA) under any federal or state legislation for family medical leave, this form sets forth the terms of a LOA that does not guarantee a job to the employee at the end of the leave.

  • Offer Letter (Since the Offer Letter is sold separately as well as part of the Starter Kit, see the separate listing of this product for a full description.)

  • Orientation & Departure Checklist – This is a customizable checklist of 40 critical items to review with a new employee during his/her first week on the job. Issues such as assignment of keys, passwords, business cards, phone extension, I-9 verification, etc. are all listed on this sheet to be checked-off as they are completed. This list also becomes an invaluable tool for ensuring the return of company property and terminating access codes and passwords when an employee leaves your organization.

  • Overview of Federal Labor Laws – This sheet identifies the federal labor laws that an employer needs to comply with based on the number of employees in an organization. As an example, an employer with 20 or more employees needs to comply with COBRA legislation. This document also provides a summary of 4 major federal labor laws that employers unknowingly violate, the 24 labor law violations to avoid, and the penalties for non-compliance.

  • Payroll Time Record – If you don’t have a formal system (e.g., punch clocks, online record, etc.) for employees to record and sign-off on the hours they work, then use this simple form. For non-exempt employees (i.e., overtime eligible), you must keep your payroll documents such as this time record for 3 years.

  • Salary & Position Summary Record – This log is designed to summarize on one sheet all the salary/compensation changes and position title changes that an employee has received while working for you. This sheet eliminates rummaging through multiple documents to find the salary or job title changes for an employee and it eliminates all the guessing as to when the actual changes occurred. This form can be kept in each employee’s file or in a master book of Salary & Position Summary Records for all employees as a quick way to compare one employee to another.

  • Supervisor’s Confidential Discipline Worksheet – Prior to initiating any disciplinary action as recorded on the Employee Counseling Record, a supervisor must complete this form and review it with his/her immediate superior. This form requires a supervisor to respond in writing to over 10 specific questions, which are the main determinants of what is considered just cause for disciplinary action in union and non-union companies. Absent good responses to these questions, disciplinary action should be reconsidered until these questions can be answered satisfactorily.

  • Wage & Hour Salary Exemption Test – One of the biggest problems in small businesses is the misclassification of employees as Exempt From Overtime when in reality the employee should have been classified as Non-Exempt. An improper classification could set-up an employer for a 3 year back pay award if an employee files a claim with the Department of Labor. This form provides you with the test for classifying an employee as Exempt. All employees who do not pass this test should be classified as Non-Exempt and therefore eligible to receive overtime.

  • Workers Compensation Illness & Accident Report – This form is an absolute must for capturing information from an employee and witnesses to a Workers Compensation injury or illness situation.
